UDID Card Apply Online Form 2024

In order to apply online for Unique Disability ID, one must follow the process mentioned below:-

Firstly visit the Unique Disability ID official website at https://swavlambancard.gov.in/

At the homepage, click at “Apply for UDID” link or directly click https://www.swavlambancard.gov.in/Applyudid
Upon clicking the link, the page for UDID Card apply online will open.

Select any of the 4 options and click “Submit” button. For eg- we have selected “I have never obtained any Disability Certificate/UDID Card issued through UDID portal or any other platform/means” and clicked “Submit” button.
Accordingly, the UDID Card registration form 2024 will appear as shown below:-

Enter personal details (name, mobile no, email ID, date of birth, gender, category), proof of identity card, address of correspondence, disability details, upload photo, signature/thumb/other print and enter other asked details accurately.
Finally, click at “Submit” button to submit UDID Card apply online form.

UDID Card Download Application (PwD Login)

Firstly visit the Unique Disability ID official website at https://swavlambancard.gov.in/
At the homepage, click at “PwD Login” link or directly click https://swavlambancard.gov.in/login to get more details about your application
On clicking the link, PwD Login page would get displayed on your screen.

Enter enrolment number/UDID number, date of birth, captcha and click “Login” button. Then dashboard displaying UDID application status will open.

Through this dashboard, one can update personal profile, download application or download receipt.
If we click at “Download Application” option, then UDID Card PwD application will get downloaded on your laptop/PC/smartphone.
