Applicant shall fill up the form completely. Applicants shall submit their Identity Card, Aadhara Card, Proof of age, Proof of EWS category, Passport Photo (2 nos.) along with application.
Applicants shall submit any one of the documents as proof of their address/age/ identity/ category as prescribed by Govt. of India/ Govt. of Odisha. Besides, MGNREGS Job card/ Antyodaya/ Arnapurna/ Old age pension/ Widow pension/Ration card under NFSA, etc. in which name and age are clearly mentioned, shall be acceptable as EWS Category.
The attendant shall submit the 50% of the travel cost in shape of Bank Draft drawn in favour of Director, Tourism payable at Bhubaneswar in any scheduled bank.
